Change of name

If you have changed your name, please return your current registration certificate, by recorded delivery, with your original marriage certificate, decree absolute or copy of your deed poll document, to the Registration Section.  You can call into the Society with these documents in person.  There is no fee for a change of name.

Lost or stolen certificate

If your certificate has been lost or stolen you will need to provide written notification to the Society and pay a fee £100 for a replacement certificate.

Changing current certificate

We are able to provide certificates in A4 size only. If you want to change your old A3 certificate to A4, please return the certificate (by recorded delivery) to the Registration Section. The cost of a replacement certificate is £100 (cheque made payable to The Pharmaceutical Society of Northern Ireland).  Your new certificate will be sent to your registered address by recorded delivery.
