Registration of temporary pharmacy premises

Please complete all sections of the form and submit 2 months before the proposed opening date of the temporary premises. The Society will pass these documents to the Pharmaceutical Inspectorate in the Department of Health and Social Services and Public Safety, who will contact the applicant with a date to inspect the premises.
